The Keeping Bristol Safe Partnership (KBSP) has today (25th March 2026) published a Safeguarding Adults Review (SAR) which was commissioned following the death of Michael (pseudonym).

The Partnership wishes to extend their sympathy to anyone affected by Michael’s death and thank those who have contributed to this review.

It is important that lessons are learned from Safeguarding Adult Reviews. A learning briefing has been created for professionals to use in individual learning, team meeting briefings or individual supervisions to ensure that the findings are disseminated widely. The SAR focused on several overarching safeguarding themes: safeguarding, risk and mental capacity; cuckooing and exploitation; self-neglect; and information sharing, partnerships and collaboration.
